My name is Dr. Florencia Halperin and I am currently serving as Chief Medical Officer of FORM. FORM is a virtual obesity medicine clinic that provides personalized care from an expert medical and dietitian team, empowering patients to lose weight to improve their health. I joined FORM in 2019 to define and implement our clinical care model based on evidence-based guidelines. It's been such a privilege to build our team of world-class board certified physicians, advanced practice professionals, and registered dietitians. I oversee the clinical team and ensure we consistently and without exception deliver high-quality and compassionate medical care to people living with obesity. We are thrilled to be expanding access to personalized expert obesity care across the U.S. by serving patients in 49 states.I am board certified in endocrinology and a member of many professional associations including the American Medical Association, Endocrine Society, and The Obesity Society. I've been honored to be recognized at a national level for my dedication to patient care (Castle Connolly America’s Top Doctors, the Schwartz Center Award for Compassionate Care).My team is hiring!
