I am a fast learner and determined to give and show my full potential within a team. I have shown that I can organise and priorities my workload to suit the need. I do have a high sense of responsibility and can work hard and calm under extreme pressure. I do have excellent people skills and can communicate on many different levels and with a variety of people with all abilities. I have been communicating with all levels of staff and external agencies within my previous roles. I am always very professional and confident in what I have communicated. I have had to work on my own initiative; however, I do work well within a team. When working within the Discharge Team I have always had to work to deadlines each day to ensure safe and timely discharges within the Hospitals I have worked in to ensure a continuous flow within the Hospital. Whilst working within the Discharge Team, Part of my role was to complete full acre need assessments for the patients to ensure they receive the right care once they leave hospital and the correct discharge pathway for everyone. Throughout my employment history I have completed Mental Capacity Assessments and triaging and raising Safeguarding and Vulnerable Adult alerts. I must also ensure that the patient and their families are sign posted to correct support for when they do leave hospital and to ensure they receive the support at home or in placement. No matter what position I have been in the patient, client or service user and their families/ carer’s have been priority. I also feel that all my skills are transferrable to be able to adapt to any situation needed
