QUALIFICATIONS: ● A seasoned LPN for 15 years with various experiences in different settings such as in a Long Term Care, Clinical, Home health, Hospice, Triage, and Hospital Setting ● With extensive experience in wound care treatment; medication administration; assess and treat patients with chronic and acute health problems ● Proficient in wound vac and tube feedings ● EMR documentation- PCC, EPIC, and Matrix ● BLS, NIOSH, COHC, BAT, UDS, IV and Phlebotomy Certified ● A compassionate healthcare provider who provides direct patient care to patients/ residents ensuring quality living ● Has an active LPN license in Washington, Wisconsin, and Illinois
